Abstract
This erratum corrects two errors in the original paper.

This article [J. Biomed. Opt. 21(7), 071109 (2016)] was originally published online on 1 March 2016 with two errors, described below.

On p. 3, there is an error in a value in the Mueller matrix. In column 2, row 2, 0.100 has been changed to 1.000 as shown:

[0.9970.0000.0010.0050.0041.0000.0100.0010.0010.0040.9910.0020.0010.0020.0090.999]

In addition, Fig. 1 showed an extra component, the quarter-wavelength plate (QWP), which has been removed from the diagram.
